CrossOver is a health home to patients, volunteers, and employees.

CrossOver Healthcare Ministry is a 501(c)(3) non-profit organization that provides compassionate, high quality healthcare to the uninsured and those enrolled in Medicaid in our community through the operation of two healthcare clinics, one in the City of Richmond and one in Henrico County.

CrossOver offers comprehensive healthcare services to its patients including primary care, specialty care, pediatrics, HIV/AIDS testing and care, women’s health and prenatal care, mental health, dental, eye and vision care; medications and medications management, and our Bridges to Health program. PRIMARY CARE

Our patients can receive a full range of primary care services at CrossOver. Patients’ primary care providers can also connect them to our other programs, including dental, vision, specialty care, and social work.

ELIGIBILITY: This program is open to CrossOver patients who are uninsured or enrolled in Medicaid.

SPECIALTY CARE

CrossOver offers a variety of specialty services to our patients, both through in-house care and referrals. Thanks to generous volunteer support, CrossOver provides over ten specialty services in-house, including pulmonology, cardiology, neurology, and many more. When CrossOver does not have an available on-site specialist, the patient is referred through Access Now to see a specialist in the community.

ELIGIBILITY: In-house specialists care for CrossOver patients who are uninsured. Medicaid patients receive referrals for care as needed.

PEDIATRICS

CrossOver’s pediatrics program utilizes our integrated care model to quickly provide children with the healthcare and wraparound services they need to thrive. CrossOver is the only charitable clinic in the greater Richmond area to provide a dedicated pediatrics program to uninsured children. We also serve children with Medicaid.

Pediatric patients receive age-appropriate vaccines at primary care appointments along with culturally competent health education designed for the entire family. Children are also fast-tracked to receive on-site preventative and emergency dental care as well as trauma-informed case management services through our social work program. 

ELIGIBILITY:  Open to CrossOver patients who are uninsured or enrolled in Medicaid with the following age requirements:

Henrico Clinic: Ages three (3) and up

Richmond Clinic: Ages 14 and up

HIV/AIDS CARE AND STD TESTING

CrossOver offers full-spectrum HIV/AIDS care, and our services include:

  • Full care for HIV/AIDS patients under the Part B Ryan White program. This program is located on the second floor at our Henrico Clinic after recently relocating from Richmond Clinic.
  • Rapid Start, a program intended to provide immediate HIV care and initiation of medication at the time of HIV diagnosis.
  • STD Community Testing program. We offer free, confidential testing for HIV, gonorrhea, syphilis, chlamydia, hepatitis B and hepatitis. You can access testing at either of our clinic locations, whether or not you’re a CrossOver patient. Walk-in testing is offered Monday through Thursday between 8:30 a.m. and 3:30 p.m., and Fridays between 8:30 a.m. and 12 p.m. Calling ahead to make an appointment is encouraged. We also run clinics for after-hours testing. Please call for more information.
  • PrEP (pre-exposure prophylaxis). Please call for more information.

ELIGIBILITY: This program is open to CrossOver patients who are uninsured or enrolled in Medicaid. Testing is available to anyone at either location whether or not you are a patient with CrossOver. 

WOMEN’S HEALTH AND OB SERVICES

CrossOver offers full women’s annual exams, including referrals for mammograms.

Our OB program offers:

  • Full prenatal testing
  • Prenatal and postpartum care
  • Registration with WIC (Women, Infant and Children benefits) and other assistance programs

We work with area hospitals to connect our patients to labor and delivery tours and classes. If a mother faces a complicated pregnancy, she is transferred to specialty care.

CrossOver OB program mothers labor and birth at the hospital they choose. Following delivery, they return to CrossOver for postpartum care.

ELIGIBILITY: This program is open to CrossOver patients who are uninsured or enrolled in Medicaid.

MENTAL HEALTH

The mental health program at CrossOver serves a diverse population though a comprehensive, team-based approach designed to support the individual completion of goals related to physical, mental, spiritual, and emotional recovery and wellness.

We offer short term counseling and long term counseling at both clinic locations through both traditional and telehealth service delivery models.

We also offer care coordination to services linking patients to additional resources in the community such as medication management services, peer support programs, psychological evaluations, and other specialized mental health programs on a case by case basis.

ELIGIBILITY: This program is open to CrossOver patients who are uninsured or enrolled in Medicaid.

DENTAL CARE

CrossOver’s dental program offers an array of dental services to our patients, including:

  • Preventative care: cleanings, x-rays, fluoride, patient education, screenings
  • Restorative: fillings, extractions and limited specialty referrals and oral surgery procedures

CrossOver’s dental staff collaborates with our primary care providers to help patients better understand the connection between oral health and their overall health.

ELIGIBILITY: This program is open to uninsured and Medicaid recipients who are also established primary care patients with CrossOver. We do not offer dental care to individuals who are seeking only dental services. Dental patients must have at least one (1) in person primary care visit with our staff clinicians each year in order to remain eligible for dental services. 

EYE CARE

CrossOver eye care patients receive a complete eye exam including retinal screenings, refractions, and visual field exams. If these routine exams reveal abnormalities, the patient will be referred to specialists for treatment. For patients needing corrective lenses, we provide vouchers for glasses at local optical shops.

Our eye care team sees patients from CrossOver as well as six other local medical safety-net providers.

ELIGIBILITY: This program is open to CrossOver patients who are uninsured. Medicaid patients receive referrals for care.

PHARMACY

CrossOver is the only charitable clinic in the Richmond area with a licensed on-site pharmacy.

Medications are dispensed to CrossOver patients as well as patients at Health Brigade, Center for Healthy Hearts, the Free Clinic of Powhatan, Bon Secours Care-A-Van, St. Joseph’s Outreach Clinic, and GoochlandCares. Medications are couriered from CrossOver to other clinics by the Bon Secours Health System lab couriers.

ELIGIBILITY: This program is open to CrossOver patients who are uninsured. Medicaid patients receive prescriptions to be filled at an outside pharmacy of their choice.

CASE MANAGEMENT

Comprehensive healthcare means more than simply tending to medical needs. Our case management team serves patients who need support with:

  • Housing insecurity
  • Food insecurity
  • Unemployment or underemployment
  • Legal issues
  • Government systems navigation

The team provides care coordination and comprehensive case management involving complex problems such as:

  • Domestic violence
  • Housing crises
  • Mental health
  • Other difficult situations

HOSPITAL DISCHARGE CLINIC

CrossOver offers a weekly hospital discharge clinic (also known as Bridges to Health) to link new patients with resources necessary to manage their health after an unplanned hospital stay. Our team-based, comprehensive care model includes an intake appointment, a social work visit for case management, health education with a nurse, and a primary care provider visit to discuss care, medications, and referrals. We also provide prescription medication at CrossOver or through an outside pharmacy. The accelerated patient intake promptly meets the complex needs of patients and provides access to ongoing, preventative primary care to prevent hospital re-admissions.
