Good to Know!
To become a CrossOver patient, you need to be:
- Eligible for Medicaid / enrolled in Medicaid
OR
- Uninsured and at or below 200% of the federal poverty level.

We accept all Virginia Medicaid insurance plans, but we do not accept patients with Medicare Part B or any other private health insurance.

IF YOU ARE UNINSURED AND DO NOT HAVE MEDICAID
Uninsured patient household income must be at or below 200% of the federal poverty level (FPL). Call 804-655-2794 to set up a financial screening appointment to determine your eligibility. Please note that appointments for uninsured patients are in high demand and it may be several weeks before a new patient/financial screening appointment is available.

IF YOU ALREADY HAVE MEDICAID
We accept all Virginia Medicaid insurance plans. We do not accept patients with Medicare Part B or any other private health insurance.
Complete the Medicaid Patient Online Registration. Please fill out one form for each member of your family who would like an appointment. CrossOver staff will respond to your request within five business days. If you prefer to download and print your registration form, you can get it here.
