CrossOver is a health home to patients, volunteers, and staff

 

CrossOver Healthcare Ministry provides compassionate, high quality healthcare to the uninsured and medically underserved in our local community through the operation of two healthcare clinics; one in the City of Richmond and one in Henrico County. CrossOver is a 501(c)(3) non-profit organization that offers comprehensive healthcare services to its patients, including: primary care, specialty care, dental, vision, behavioral health, HIV/AIDS care, OB/GYN, medications and medications management, and social work case management. PRIMARY CARE

Our patients can receive a full range of primary care services at CrossOver. Patients’ primary care providers can also connect them to our other programs, including dental, vision, specialty care, and social work. Both uninsured and Medicaid patients can receive care at CrossOver if they meet eligibility requirements.

PEDIATRICS

CrossOver’s pediatrics program utilizes our integrated care model to quickly provide children with the healthcare and wraparound services they need to thrive. CrossOver is the only charitable clinic in the greater Richmond area to provide a dedicated pediatrics program to uninsured children. We also serve children with Medicaid. 

SPECIALTY CARE

CrossOver offers a variety of specialty services to our patients, both through in-house care and referrals. Because of the generous volunteer support, there are over ten specialty services provided in-house at CrossOver, including pulmonology, cardiology, neurology, and many more. When CrossOver does not have a specialist on-site available, the patient is then referred through Access Now to see a specialist in the community.

DENTAL

CrossOver’s dental program offers an array of dental services to our patients, including:

  • Preventative care: cleanings, patient education, screenings
  • Restorative: fillings, extractions
  • Dentures

CrossOver’s dental program collaborates with our pharmacists and primary care providers to help patients better understand the connection between oral health and their overall health. 

VISION

CrossOver vision patients receive vision services such as:

  • Complete eye exams including retinal screenings
  • Refractions
  • Visual field exams.
  • Specialty referrals
  • Glasses vouchers

Our vision team sees patients from CrossOver as well as seven other medical safety-net providers.

PHARMACY

CrossOver is the only charitable clinic in the Richmond area with a licensed on-site pharmacy. Medications are dispensed to CrossOver patients as well as patients at The Health Brigade, Center for Healthy Hearts, the Free Clinic of Powhatan, Bon Secours Care-A-Van, St. Joseph’s Outreach Clinic, and Goochland Free Clinic and Family Services. Medications are couriered from the Community Pharmacy to other clinics by the Bon Secours Health System lab couriers.

WOMEN’S HEALTH

CrossOver offers:

  • full women’s annual exams, including referrals for mammograms.
  • Our OB program offers
  • full prenatal testing
  • OB care
  • registration with WIC

We work with area hospitals to connect our patients to labor and delivery tours and classes at the hospital they choose. If a mother faces a complicated pregnancy, she is transferred to specialty care. CrossOver OB program mothers labor and birth at the hospital they choose. Following delivery, they return to CrossOver for postpartum care.

HIV/AIDS CARE

CrossOver is the only charitable clinic in Richmond to offer full-spectrum HIV/AIDS care. We offered:

  • Walk-in, rapid confidential HIV testing

Testing is available to patients as well as non-patients Monday through Friday, 9 a.m. to 4 p.m., and after hours testing is available on certain dates. The test requires a finger stick and results are available in 15 minutes. Call 804-655-2794, ext. 115 for further information.

MENTAL HEALTH

At CrossOver, we know that mental health is a key part of comprehensive healthcare. At both CrossOver clinics, we offer:

  • short-term counseling
  • long-term counseling
  • psychiatric care

SOCIAL WORK

Comprehensive healthcare means more than simply tending to medical needs. Our social work team helps patients who need support with:

  • affordable housing
  • food insecurity
  • unemployment or underemployment
  • legal issues
  • navigating a variety of government systems

BRIDGES TO HEALTH

CrossOver offers a weekly hospital discharge clinic to link new patients with resources necessary to manage their health after an unplanned hospital stay. The team-based, comprehensive care model includes an intake appointment, social work visit for case management, health education with a nurse, primary care provider visit to discuss care, medications and referrals, and fill prescriptions.  The accelerated patient intake promptly meets the complex needs of patients and provides access to ongoing, preventative primary care to prevent hospital re-admissions
