Thanks for trusting CrossOver with your health needs. The patient intake process can be difficult, but we’re here to help every step of the way. We would love to take every patient in need, but our capacity is limited. Please follow the steps below to begin the process.

CHECK ELIGIBILITY

Before your first medical appointment at CrossOver, you must complete eligibility forms and a financial screening. To qualify as a new non-Medicaid patient at CrossOver, patients must be at or below 200% of the federal poverty level and have no form of insurance. If you have Medicaid or may be eligible for Medicaid, please click here.

To qualify to be a non-Medicaid patient at Crossover, your income must be at or below the following levels:

GET IN TOUCH

Are you pregnant? HIV positive? Been in the emergency room or hospital within the last 30 days? Or is the new patient a child? If YES, please call 804-655-2794 to inquire about becoming a new patient. You may not have to go through the full intake process. If you have Medicaid, please visit this page.

Henrico Clinic

If you are interested in becoming a new patient at the Henrico Clinic (8600 Quioccasin Rd.), please call 804-655-2794 on the second or fourth Friday of every month at 2 p.m. At that time, we will take as many new patients as we can. If you receive a new patient spot, we will give you a date and time within two weeks to drop off your documents for financial screening (see step three, below).

Richmond Clinic

To become a new patient at the Richmond Clinic (108 Cowardin Ave.), please come to the clinic on any Monday, Tuesday, Wednesday, or Thursday, between 9 a.m. and 3 p.m. Please bring your financial screening documents with you at that time (see step three, below).

COMPLETE YOUR FINANCIAL SCREENING

Henrico Clinic

Those selected on new patient intake days (see step two, above) will be given a date and time within two weeks to drop off their documents for financial screening. These document drop-off appointments usually will be held on Tuesday and Thursday afternoons. CrossOver staff will also give specific instructions about what documents to bring to these appointments.

Please review the application which provides the required forms of documentation needed.

When you come for your financial screening documents drop-off appointment, please have a photo ID and proof of income for all income earned by your household, as directed by CrossOver staff during your intake phone call. If you miss your financial screening appointment or do not bring the correct documents, your appointment cannot be rescheduled. You will have to return to step two.

Once financial screening is complete, patients can schedule their first appointments. During the COVID-19 outbreak, most patient appointments are being held by phone or video.

Richmond Clinic

When you come to the clinic to become a new patient, please have a photo ID and one proof of income for all income earned by your legal household. 

Please note that additional documentation may be required depending on your financial situation.

Please review the application which provides the required forms of documentation needed. CrossOver staff will review your documents and contact you if further information is needed.

Once financial screening is complete, patients can schedule their first appointments. During the COVID-19 outbreak, most patient appointments are being held by phone or video.
