In order to be seen at a CrossOver clinic, you must first complete the eligibility forms and make an appointment at either the Western Henrico or Cowardin Avenue location to undergo a screening appointment. Patients must be at or below 200% of the federal poverty guideline and have no form of insurance (private, Medicare or Medicaid.)
Please review the "Income Guidelines for Patients" section which provides the income limits as well as the required financial documentation. Download and complete the eligibility packet and call either the Western Henrico Clinic (804.622.0803) or Cowardin Avenue Clinic (804.233.5016) to schedule a screening appointment.
