In order to be seen at a CrossOver clinic, you must first complete the eligibility forms and undergo an IN-PERSON financial screening. DO NOT MAIL, FAX OR EMAIL THESE FORMS. 

Patients must be at or below 200% of the federal poverty level & have NO form of insurance (including Medicare, Medicaid & private.) Please review the "Income Guidelines for Patients" section which provides income limits as well as the required forms of documentation.

Financial Screening Times 

Unfortunately, we are not able to accept all new patients seeking healthcare services. We offer screenings on a first-come, first serve basis at the times and locations listed below. Times & days are subject to change without notice. We reserve the right to limit the number of individuals screened in at any given time. If we are not able to give you a screening on the day you come in, and we have spaces on our waiting list and you have all necessary paperwork, we will give you a voucher and a date to return. Please know that there may be a wait for the next available screening. We will do our best to accommodate everyone, and we thank you for your patience and understanding.

Due to the volume of people requesting our services, people arriving after this time will be told to come back another time.  Please be prepared to wait several hours if granted an interview.

Patients renewing eligibility will be seen between 1:30 and 3:00 on Monday & Wednesday.

Confidential Rapid HIV Testing

HIV testing is now available on a walk-in basis at our Cowardin location.  You do not need to be a CrossOver patient in order to use this service. The test requires a fingerstick or blood draw and results are generally available within 20 to 30 minutes.  Testing is available Monday through Thursday between 9AM--4PM. After hours testing are available on certain dates. Call 804-233-5016 ext. 130 for further information.
